Ensocare and Ceras Health Announce Value Added Reseller Agreement
Partnership Brings Wearables, Remote Monitoring Capabilities and Enhanced Features to Care Coordination Solutions
Omaha, NE and New York, NY – February 15, 2017 — Ensocare, a CQuence Health Group company, today announced a value added reseller agreement with Ceras Health, a provider of healthcare technology and software services. Through this agreement, Ceras Health’s patent-pending software-as-a-service will supplement Ensocare’s Sync and Connect technology platform, which serves to engage all members of a patient’s care plan through education, remote monitoring, and early intervention.
“This partnership with Ceras Health delivers powerful features and capabilities to Ensocare’s existing Sync and Connect solutions,” said Luis Castillo, Ensocare President and CEO. “We’ve already been in discussions with customers excited about the new features, such as wearable technology and remote biometric monitoring, which will allow care teams greater oversight after a patient leaves the hospital, thereby leading to reduced readmissions and improved patient outcomes.”
“We are very excited to enter this agreement with Ensocare as it further extends our presence in the US Acute Healthcare Market,” said Kevin Murphy, CEO and Chairman of the Board of Ceras Health. “Our technically advanced and patient-centric SaaS platform will add value to their existing client service offerings by improving care coordination and increasing efficiencies for providers while engaging and motivating patients.”
Ensocare’s software engines also include: Transition, designed to automate the care transition between hospitals and post-acute facilities; and Insight, which provides robust data and predictive analytics to allow for the identification of trends, the discovery of opportunities for improvement, and oversight of the post-acute network.
Ceras Health’s patent-pending SaaS platform, I’M HOME!®, enables patient engagement and supports care coordination to reduce hospital readmissions and health system utilization. Through I’M HOME!®, Ceras Health connects patients post-discharge and keep them engaged in and adhering to their care plans, resulting in lower costs, improved outcomes and reduced labor for the clinical staff.
